Order
To place your order, proceed as follows:
- Open the forms
2. Form "Data of Health Care Facility"
opt. Credit Card Authorization (we prefer bank transfer)
- Fill in the PDF form at your computer
- Store the PDF form (input will be stored) - as a backup
- Print the PDF
- Sign the print
- Repeat for all requested forms
- Send all printed forms via fax or regular mail - safe way for sensible data
- Alternative: Email all PDFs (the signed sheets as SCAN) - less safe, not recommended for credit card data
